Eligibility criteria are in place to ensure alignment with patients' needs and health goals for an effective path to recuperation.

  • A patient must be hospitalized as an Acute Care Inpatient for a minimum of three consecutive midnights within the last 30 days, and your Swing Bed care must be related to that acute hospital diagnosis.
  • A doctor has determined a need for daily skilled care.
  • Patients must be willing to participate, and show continued progress, in the daily skilled care program.
  • Referrals for Swing Bed can come from any hospital.
  • Patients must have hospital-based skilled benefits through their primary insurance.

Personalized Swing Bed Care Plans

The duration of a patient's stay in Wilson Medical Center's Swing Bed program depends on your unique care plan, goal achievement, and discharge criteria. An average swing bed stay ranges between 1-2 weeks but can extend up to 100 days in uncommon circumstances.

At Wilson Medical Center, our focus is nurturing your functional, nutritional, and medical well-being. Our proficient team collaborates closely with you and your family to formulate a discharge plan and set health goals, ensuring you maintain independence during your stay.
